If you live in Melbourne and want generic Cialis, the medicine you are looking for is tadalafil. It works well for most men with erectile dysfunction, it is cheap as a generic, and it is legal to buy, but only with an Australian prescription, whether you buy it at a pharmacy on Chapel Street or import it from overseas.

Prescription first: the rule that applies in Victoria

Tadalafil is listed in Schedule 4 of the national Poisons Standard, the category for prescription-only medicines. In Victoria that classification is applied through the Drugs, Poisons and Controlled Substances Act 1981. The result is the same as interstate: a pharmacist can only supply tadalafil against a prescription, and the TGA's personal import rules require you to hold a valid Australian prescription when an overseas parcel arrives.

The TGA spells out the consequence of skipping this step. Its example of someone buying a prescription medicine from an overseas website without a script is described as non-compliant, unlawful and liable to seizure by customs.

Why so few men ask for help

The Men in Australia Telephone Survey (MATeS) was run by Andrology Australia at Monash University in Melbourne. It interviewed 5,990 men aged 40 and over across every state in 2003. Erectile dysfunction was the most common reproductive health problem it measured, with an age-standardised prevalence of 21%, ahead of significant urinary symptoms and prostate disease.

The more telling finding was about behaviour. Only 30% of men with erectile dysfunction had sought medical help. Willingness to seek help varied with age and ethnic background. Many men, in other words, live with a treatable problem for years because the first conversation feels awkward.

Talking to a doctor in Melbourne

The conversation is shorter than most men expect. A GP will ask how long the problem has been present, whether morning erections still occur, what medicines you take, and about your heart, blood pressure, blood sugar and mood. They may order blood tests for glucose, cholesterol and testosterone. Erectile problems can show up years before heart disease does, so this check has value beyond the prescription.

Choosing between 5 mg, 10 mg and 20 mg

The US Cialis label sets the as-needed starting dose at 10 mg, taken before sex, with the option to go up to 20 mg or down to 5 mg. That flexibility is the point: there is no prize for starting high. Men with moderate kidney impairment start at 5 mg, and men with mild or moderate liver impairment should not exceed 10 mg. Men taking strong CYP3A4 inhibitors are limited to 10 mg once every 72 hours.

Side effects by dose: what the trials show

The label pools eight placebo-controlled trials of as-needed tadalafil. Headache and indigestion clearly rise with dose: headache from 5% on placebo to 15% on 20 mg, and indigestion from 1% to 10%. Back pain, muscle aches and flushing stayed at 6% or less at every dose.

So if 10 mg works for you, there is a real side-effect reason to stay there. If 10 mg is not enough, 20 mg is the next step, and the trade is roughly a few more percentage points of headache and indigestion. The back and muscle aches typical of tadalafil usually start 12 to 24 hours after a dose and settle within 48 hours.

How long tadalafil lasts and when to take it

The label states that as-needed tadalafil improved erectile function compared with placebo for up to 36 hours after a dose. It can be taken with or without food. It does not cause an erection on its own: sexual stimulation is still needed. Take no more than one dose in 24 hours. Pack size and the three-month cap

Bigger packs are cheaper per tablet, but for Australia the limit is three months' supply per order. With tadalafil at one tablet a day maximum, that is about 90 tablets. Tadalafil and alcohol both widen blood vessels. The label warns that five or more standard drinks with tadalafil can cause dizziness, a fast heart rate and a drop in standing blood pressure. Alpha blockers used for prostate symptoms and other blood pressure tablets add to the effect, so your doctor may start you on the lowest dose.

Spotting counterfeit or adulterated products

The TGA has issued many safety alerts about "natural" erection products found to contain undeclared sildenafil or tadalafil. A product with an undeclared ingredient is counterfeit and cannot be lawfully imported. A genuine generic shows the manufacturer's name, the active ingredient and strength, a batch number and an expiry date on the pack.

If tadalafil does not work

Give it a fair trial: several attempts, at an adequate dose, with sexual stimulation. If it still does not work, go back to your doctor. Options include adjusting the dose, switching to a daily regimen, trying another medicine, checking testosterone, or referral to a urologist.
