The rules in South Australia

South Australia controls prescription medicines under the Controlled Substances Act 1984 and its poisons regulations, which follow the national Poisons Standard. Tadalafil is Schedule 4. A pharmacist in Adelaide can supply it only on a valid prescription, and the same prescription is what makes a personal import from overseas lawful.

What the Florey Adelaide Male Ageing Study found

The Florey Adelaide Male Ageing Study (FAMAS) recruited randomly selected, community-dwelling men from Adelaide's northern and western suburbs. In an analysis of 1,195 of them, published in the Journal of Sexual Medicine in 2012, the prevalence of erectile dysfunction measured with the International Index of Erectile Function was 17.7%. Low desire for sex with a partner affected 13.5%. Low solitary sexual desire was much more common, at 67.7%, which mostly reflects how the questionnaire scores solo desire rather than a problem in itself.

The study linked erectile dysfunction with a range of health factors, and all the sexual problems it measured were associated with depression. The practical point for Adelaide men: ED is often part of a wider health picture, and a GP visit can pick up things a tablet will not fix.

South Australia's earlier community study

Before FAMAS, a 1997 household survey of South Australian men over 40, published in the Medical Journal of Australia in 1999, was the first Australian study to measure ED in the general community rather than in clinic patients. It found that cardiovascular risk factors predicted erectile dysfunction in older men, and that most men aged 70 to 79 reported sex less often than they considered normal for their age.

The US Cialis label approves both patterns. As needed: 10 mg before sex to start, adjusted to 20 mg or 5 mg, with effect lasting up to 36 hours and no more than one dose a day. Once daily: 2.5 mg to start, increased to 5 mg if needed, taken at roughly the same time every day without timing it to sex.

• As needed suits men who have sex once or twice a week, like taking as few tablets as possible, and can plan a little.
• Daily suits men who have sex more often, want to stop thinking about timing, or also have urinary symptoms from an enlarged prostate.

Daily tadalafil and side effects

Because the daily dose is small, side effect rates in the label's 12-week once-daily trials were low and mostly within a few points of placebo. Headache was reported by 6% of men on 5 mg daily against 5% on placebo, and indigestion by 5% against 2%.

The trade-off is that daily dosing keeps tadalafil in the blood all the time. The label notes this when weighing interactions with nitrates, alpha blockers, blood pressure medicines, strong CYP3A4 inhibitors and heavy drinking.

Daily tadalafil for an enlarged prostate

Tadalafil 5 mg once daily is approved for the urinary symptoms of benign prostatic hyperplasia (BPH), and for men who have both BPH and erectile dysfunction. The label says it is not recommended together with an alpha blocker for BPH, and that other urological conditions, including prostate cancer, should be considered before starting.
