Generic Advair and Seroflo: What the Strength Numbers Mean - pharmacy guide
Advair, its US generics and Cipla's Seroflo all contain the same two medicines: fluticasone propionate, an inhaled steroid, and salmeterol, a long-acting beta agonist (LABA). The products are sold in sprays, powder discs and capsules, and the number on the box does not mean the same thing across them. Seroflo 250 Rotacaps and Seroflo 250 inhaler sound like the same strength. At their labelled doses, the inhaler gives twice as much steroid a day.
This guide decodes the strengths, matches each Seroflo product to its US Advair equivalent, explains which strength the US label uses for COPD, sets out the safety evidence on salmeterol, and compares US pharmacy costs for brand and generic. Every figure comes from the current US prescribing information, the UK and Australian regulator records, or the CMS drug cost survey.
The short answer
Generic Advair Diskus has been sold in the US since 2019, starting with Wixela Inhub, and it costs pharmacies about 40% of the brand. Seroflo is Cipla's Indian brand of the same combination. Seroflo Rotacaps and Ciphalers are written the way Advair Diskus is written (fluticasone per dose, with salmeterol 50 mcg), and are taken as one inhalation twice a day. Seroflo inhalers (sprays) are written per puff, with salmeterol 25 mcg, and are taken as two puffs twice a day. So Seroflo 250 Rotacaps matches Advair Diskus 250/50, while Seroflo 250 inhaler matches Advair HFA 230/21, which is the highest Advair HFA strength.

What is in Advair and Seroflo
Fluticasone propionate is a corticosteroid that reduces inflammation in the airway lining when inhaled. Salmeterol relaxes the airway muscle for about 12 hours. Neither works fast enough to relieve a sudden attack, which is why every one of these products is a twice-daily preventer and why you still need a separate reliever such as albuterol (salbutamol), sold in India as Asthalin, or Levolin (levosalbutamol).
GSK's Advair comes in two US devices: Advair Diskus, a dry powder disc with 60 blisters, and Advair HFA, a pressurised spray with 120 puffs. Outside the US the same products are called Seretide Accuhaler and Seretide Evohaler. Cipla's Seroflo range covers both styles: Rotacaps (powder capsules for a separate Rotahaler or Revolizer device), Ciphaler and Multihaler (pre-loaded powder devices), and pressurised sprays, including a breath-actuated Synchrobreathe version.
Generic Advair in the US
The FDA approved Mylan's Wixela Inhub, the first generic of Advair Diskus, on 30 January 2019. FDA records now list approved generic fluticasone and salmeterol powders from Mylan, Hikma, Teva, Respirent and, since 27 July 2026, Cipla, the company behind Seroflo. Cipla's US product is a Diskus-style powder in 100, 250 and 500 mcg strengths with salmeterol 50 mcg per blister. Prasco also lists generic-labelled versions of both Advair Diskus and Advair HFA on DailyMed.
An FDA-approved generic inhaler has met the FDA's equivalence requirements against the brand. The device can look and open differently from a Diskus, so ask the pharmacist to show you how it opens and loads if you are switched.
Why the same number means different doses
Two conventions cause the confusion.
First, powders and sprays are labelled per different units. A Diskus blister, a Ciphaler blister and a Rotacap each hold one full dose: one inhalation, twice a day. A spray puff holds half a dose: the label says two puffs, twice a day. So the per-puff number on a spray has to be doubled before you compare it with a powder.
Second, US spray labels state the dose leaving the mouthpiece, while most other countries state the dose leaving the valve. The Advair HFA label says each puff releases 50, 125 or 250 mcg of fluticasone and 25 mcg of salmeterol from the valve, of which 45, 115 or 230 mcg and 21 mcg leave the actuator. That is why the same spray is called Advair HFA 230/21 in the US and Seretide 250 Evohaler or Seroflo 250 in other markets.
Put together, Seroflo 250 inhaler at two puffs twice a day delivers 1,000 mcg of fluticasone from the valve each day. Seroflo 250 Rotacaps at one capsule twice a day delivers 500 mcg. Someone who switches from Rotacaps to the spray "at the same strength" doubles their inhaled steroid without knowing it. Someone who goes the other way halves it.
Seroflo 250 inhaler is a high-dose product
The UK licence for Cipla's twin product, Sereflo 25/250, sets the adult dose at two puffs twice a day, recommends a spacer for this strength, and licenses both Sereflo strengths for adults aged 18 and over only, not for children or teenagers. The US label sets Advair HFA 230/21 two puffs twice a day as the maximum Advair HFA dose. That does not make it wrong for the people who need it. It means it is not a starting strength, and nobody should move up to it because "250 sounded like what I was on".
COPD: the US label uses 250/50
Advair Diskus is approved for COPD in the US at one strength: 250/50, one inhalation twice a day. Advair HFA is approved only for asthma. In two identical one-year trials of 1,579 people with COPD, Advair Diskus 250/50 cut moderate or severe flare-ups by about 30% compared with salmeterol alone (30.5% and 30.4%), and cut flare-ups needing steroid tablets by 39.7% and 34.3%.
The higher 500/50 strength did not do better on flare-ups. No trial compared the two strengths directly, and the label notes that across trials the reduction seen with 500/50 was not greater than with 250/50. In the three-year survival trial of 6,112 people, all-cause death was 12.6% on 500/50 against 15.2% on placebo, which was not a significant difference. Pneumonia, on the other hand, rose with dose: 7% on 250/50 vs 3% on salmeterol in the one-year trials, and 16% on 500/50 vs 9% on placebo over three years (6,184 people in the safety analysis), higher again in people over 65.
For COPD, then, the Seroflo product that matches the US label is Seroflo 250 Rotacaps or Ciphaler. The Seroflo 250 spray gives twice that steroid dose and is not the US COPD regimen.
The salmeterol safety story
Salmeterol carried a boxed warning in the US for years because of one trial. In SMART, a 28-week US study of 26,355 people, salmeterol added to usual asthma care without a required steroid led to 13 asthma deaths against 3 on placebo (relative risk 4.37, 95% CI 1.25 to 15.34). The finding is treated as a class effect of LABA used without an inhaled steroid.
The FDA then required four large 26-week safety trials of steroid plus LABA inhalers against steroid alone, including one with Advair Diskus in adults and adolescents and one in children aged 4 to 11. In the combined adult and adolescent analysis of 35,089 people, serious asthma events (hospital stays of 24 hours or more, intubations or deaths) occurred in 116 on combination inhalers against 105 on steroid alone, a hazard ratio of 1.10 (95% CI 0.85 to 1.44). In children, the rate was 0.9% against 0.7%, hazard ratio 1.29 (0.73 to 2.27). Neither showed a significant increase, and in December 2017 the FDA removed the boxed warning about asthma deaths from combination inhalers.
Never use salmeterol on its own for asthma
The practical rule from SMART is that a LABA in asthma must always come with an inhaled steroid. A combination inhaler guarantees that, which is one reason they are preferred. Do not add a separate salmeterol or formoterol inhaler on top of Seroflo or Advair, and do not take extra doses when symptoms worsen. The Advair Diskus label says more than one inhalation twice a day is not recommended because some people get more side effects from higher salmeterol doses. If your reliever use is climbing, your asthma is getting worse and needs review.
Side effects
The labels list the effects you would expect from an inhaled steroid plus a beta agonist:
- Throat irritation, hoarseness and oral thrush. Rinse your mouth with water and spit after each dose.
- Headache and upper respiratory tract infections.
- Pneumonia in COPD, as above.
- Fast heartbeat, tremor, low potassium and raised blood sugar from salmeterol, mostly at higher doses.
- With long-term use: lower bone density, cataracts and glaucoma, slower growth in children, and adrenal suppression at high doses.
- Rarely, paradoxical bronchospasm: wheeze straight after a dose. Stop, use your reliever and get help.
Who should not use them
All Advair products are contraindicated as the main treatment of status asthmaticus or other acute attacks needing intensive treatment, and in people allergic to the ingredients. The powder products (Advair Diskus and its generics) are also contraindicated in severe milk protein allergy, because the lactose carrier can contain milk proteins; Advair HFA, the spray, does not carry that contraindication. If you have a milk protein allergy, check the ingredient list of any Seroflo Rotacap or Ciphaler before use.
Two warnings deserve their own line. First, if you are coming off steroid tablets, they must be reduced slowly: the Advair label reports deaths from adrenal insufficiency during and after the switch from steroid tablets to an inhaled steroid, and during illness, injury or surgery you may need extra steroid. Second, inhaled steroids lower resistance to infection, and chickenpox or measles can be serious or even fatal in people without immunity, so seek advice after any exposure.
Take extra care, and tell your prescriber, if you have heart rhythm problems, high blood pressure, seizures, an overactive thyroid, diabetes, osteoporosis, glaucoma or cataracts, tuberculosis, or untreated infections. The US label does not recommend combining these inhalers with strong CYP3A4 inhibitors such as ritonavir, ketoconazole, itraconazole or clarithromycin, because steroid and heart side effects can increase. Beta blockers can blunt salmeterol and may trigger bronchospasm in asthma. The label also advises extreme caution with MAOIs and tricyclic antidepressants, and care with loop or thiazide diuretics, which can add to potassium loss.
Children
In the US, Advair Diskus 100/50 is approved for children aged 4 to 11 whose asthma is not controlled on a steroid alone, at one inhalation twice a day. Advair HFA starts at 12. Higher strengths are for adolescents and adults. The UK licence for the equivalent Sereflo 25/125 and 25/250 sprays covers adults aged 18 and over only. Children's doses should only ever be chosen by their doctor.

NADAC is what US pharmacies pay to buy the product, from the federal weekly survey (here, the week of 7 October 2026). It is not the counter price, and insurance changes what you pay. The pattern is still clear: generic powder is the cheap route in the US; the generic spray saves much less. Teva's AirDuo RespiClick is a different fluticasone and salmeterol product with lower strengths (55/14, 113/14 and 232/14 mcg), and its strengths do not map onto Advair or Seroflo.
Seroflo outside India
Cipla's Seroflo is not only an Indian product. In the UK, Cipla EU's Sereflo 25/125 and 25/250 sprays are licensed by the MHRA and listed in the NHS dictionary of medicines alongside other branded generics such as AirFluSal, Aloflute, Combisal and Sirdupla. In Australia, the TGA has registered Seroflo Multihaler and Ciphaler powders at 250/50 and 500/50. None of the Indian-made Seroflo packs is FDA-approved; the FDA-approved Cipla product is the separate US generic described above.
Choosing a Seroflo format
- Rotacaps need a separate device such as the Rotahaler or Revolizer, and you load one capsule each time. Never swallow the capsule.
- Ciphaler or Multihaler work like a Diskus: open, load, inhale. Good for people who find sprays hard to time.
- Sprays need you to press and breathe in at the same moment. A spacer helps and is recommended in the UK for the 250 strength. The Synchrobreathe version fires when you breathe in, which removes the timing problem.

Storing and replacing your inhaler
The Advair labels give simple rules, and the Seroflo packs carry their own instructions, which you should follow. The Advair Diskus label says to keep the powder sealed in its foil pouch until you start it, and to discard it 1 month after opening the pouch or when the counter reads 0, whichever comes first; check the discard period printed on any Seroflo Ciphaler or Multihaler. Store a spray with the mouthpiece down at room temperature, never puncture or heat the canister, and discard it when the counter reads 000: after that the dose per puff is not reliable, even though the canister may still spray. Keep Rotacaps in their strip until you load one. Order your next pack before the counter runs low, since missing a few days of a preventer can let asthma control slip.
Switching from Advair to Seroflo
- Write down your current product, strength and how many puffs or inhalations you take. "Advair 250" is not enough; you need Diskus or HFA.
- Use the table at the top of this guide to find the Seroflo product with the same daily fluticasone dose.
- If you change device type, learn the new technique before your old inhaler runs out.
- Check the new product's dose counter or capsule count so you know when to reorder.
- Agree any strength change with your prescriber, especially for COPD and for children.
Mistakes to avoid
- Assuming "250" means the same dose on a spray and on a powder.
- Taking two Rotacaps per dose because the spray version uses two puffs.
- Using a high-dose spray for COPD when the label's COPD dose is 250/50 powder.
- Adding a second LABA inhaler, or using Seroflo as a reliever.
- Stopping when you feel well. Preventer inhalers keep working only while you take them.
