Generic Semaglutide in 2026: Where It Is Sold, Whether It Is Legal and How It Compares With Wegovy

Generic semaglutide became real in 2026, but only in some countries. India's patent on the molecule expired on 20 March 2026 and Indian companies launched their own semaglutide pens the next day. Canada approved its first generic in April 2026 and its first generic Wegovy for weight management in June. In the United States, the UK and the European Union, patents still block generics until at least 2031, and anything sold there as "generic semaglutide" is either the branded product, a compounded preparation, or an unapproved import.

This guide explains what generic semaglutide is, where it is legally sold, how the Indian brands compare with Wegovy and Ozempic, what the pens cost, and the practical risks of buying an injectable across borders. It draws on the current US Wegovy label, Health Canada announcements, company releases, CMS price data and FDA statements, all listed at the end.

The short answer

• India: generic semaglutide has been on sale since 21 March 2026 from several companies, including Sun Pharma's Noveltreat (weight management) and Sematrinity (type 2 diabetes).
• Canada: Health Canada has approved generic semaglutide for type 2 diabetes since late April 2026 and for weight management since 29 June 2026.
• United States: no FDA-approved generic is on the market. Patents are expected to block generics until at least late 2031, and routine compounding of semaglutide ended in 2025.
• UK and EU: patent protection is expected to last until 2031.
• Same drug? A generic contains the same active peptide and is approved as equivalent by its own regulator. The pen, the inactive ingredients and the dose steps can differ.

What "generic semaglutide" means

Semaglutide is a modified version of the gut hormone GLP-1. It lowers blood sugar, slows stomach emptying and reduces appetite, and it has a half-life of about a week, which is why it is injected once weekly. Novo Nordisk sells it as Ozempic (type 2 diabetes injection), Wegovy (weight management and cardiovascular risk, injection and tablet) and Rybelsus (type 2 diabetes tablet). In the US the diabetes tablet is now sold as Ozempic tablets; Rybelsus remains the tablet's name elsewhere.

A generic is semaglutide made by another company and approved by a national regulator as equivalent to the original. Semaglutide is a peptide with a fatty side chain that keeps it in the blood for days. The Wegovy label says Novo Nordisk makes the peptide backbone by yeast fermentation; generic makers may make it by chemical synthesis instead. Health Canada describes the generics it has approved as complex synthetic products that are pharmaceutically equivalent to the brand, and says its review checks that differences between them do not affect safety, efficacy or quality.

Three things are commonly called "generic semaglutide" but are not approved generics:

• Compounded semaglutide made by a pharmacy, often in vials, sometimes as semaglutide salts. These are not approved products.
• "Research use only" semaglutide sold online as a powder or liquid. It is not approved for human use anywhere.
• Branded pens bought abroad, such as Indian Wegovy. These are genuine Novo Nordisk products, but they are not generics and are not approved for sale in other countries.

Why India came first

Patents run country by country. Novo Nordisk's product patent on semaglutide in India expired on 20 March 2026, and on 21 March several Indian companies launched semaglutide injections, including Sun Pharma, Dr Reddy's and Glenmark. Before that, Novo Nordisk had launched Wegovy in India in June 2025 and, according to Reuters, cut its Indian price by up to 37% in late 2025 ahead of the patent expiry.

Sun Pharma's products are the best documented, because the company published its approval and launch details:

Sun says the pens were developed by global device suppliers and are manufactured in Europe. The Indian prices apply in India; importers, pharmacies and shipping add costs elsewhere.

Canada: the first G7 country with generics

Canada's semaglutide patent situation allowed generics in 2026, and Health Canada moved quickly:

Health Canada said in June that it was reviewing six more semaglutide submissions. For Canadians, that means approved, locally regulated generics with a Canadian prescription, which is the lowest-risk way to get lower-cost semaglutide anywhere in North America.

United States: no generic, and the compounding window has closed

There is no FDA-approved generic semaglutide on the US market. Apotex received a tentative FDA approval for a generic version of Ozempic in April 2026, but a tentative approval cannot be marketed while patents block it, and Novo Nordisk is suing generic applicants. US patent protection on the molecule is expected to last until at least late 2031, and Novo Nordisk holds further patents on formulations, uses and pens.

From 2022 to early 2025, many Americans used compounded semaglutide because the branded product was on the FDA shortage list, which allowed pharmacies to make copies. The FDA declared the shortage resolved on 21 February 2025. The grace period ended on 22 April 2025 for state-licensed compounding pharmacies and on 22 May 2025 for outsourcing facilities. In April 2026 the FDA proposed excluding semaglutide from the list of bulk substances outsourcing facilities may use. Compounded semaglutide is now limited to narrow patient-specific cases.

Importing semaglutide from abroad for personal use is not a legal route in most circumstances. The FDA's personal importation policy is discretionary and applies to narrow situations, and a drug approved only in India or Canada is an unapproved drug in the US. UK, EU and Australia

In the European Union, supplementary protection certificates extend the semaglutide compound patent to March 2031, and UK pharmacy sources expect protection there to last until about 2031. Until then, the only licensed semaglutide in the UK and EU is Novo Nordisk's. Is Indian generic semaglutide the same as Wegovy?

The active ingredient is the same molecule, and Sun's weight management version was approved after an Indian phase III trial. What can differ:

• Manufacturing route. Fermentation for Novo Nordisk, often chemical synthesis for generics. Regulators assess whether differences from the brand, including those from a different manufacturing route, affect safety, efficacy or quality.
• Inactive ingredients and pen. Wegovy's single-dose pens contain disodium phosphate dihydrate, sodium chloride and water, while the multi-dose FlexTouch pen contains disodium phosphate dihydrate, phenol, propylene glycol and water; a generic may use different inactive ingredients and a different pen mechanism.
• Dose steps. Noveltreat copies Wegovy's five escalation strengths (0.25 mg to 2.4 mg). Sematrinity is a dial pen like Ozempic, so how much each click delivers depends on that pen's instructions, not on Wegovy's.
• Regulator. Each generic is approved by its own country's regulator. An Indian approval is not an FDA, MHRA, Health Canada or TGA approval.

No large outcome trial has been run with a generic. The efficacy and heart-outcome data people quote, including the figures below, come from Novo Nordisk's trials of Wegovy and Ozempic. Generics rely on being equivalent to that product.

What semaglutide 2.4 mg does for weight

In Novo Nordisk's Study 2 (STEP 1), 1,961 adults with obesity, or overweight plus a weight-related condition, and without diabetes, took semaglutide 2.4 mg weekly or placebo for 68 weeks, all with diet and activity advice. Average weight change was 14.9% on semaglutide and 2.4% on placebo.

In people with type 2 diabetes the effect was smaller: 9.6% against 3.4% on placebo in Study 3. And the weight returns when treatment stops. When people who had taken semaglutide for 20 weeks were switched to placebo in Study 5, they regained 6.9% of their body weight by week 68, while those who stayed on it lost a further 7.9%. Doses and pens

Semaglutide for weight management is started low and raised every four weeks to limit nausea. Wegovy's five escalation strengths, and Noveltreat's, exist to cover this climb.

The label lets a prescriber delay a step by 4 weeks if a dose is not tolerated. If two or more weekly doses in a row are missed, it advises restarting the escalation at a lower dose. The current US label also includes a 7.2 mg dose (Wegovy HD) for adults who have tolerated 2.4 mg for at least 4 weeks and need further weight loss, and a daily Wegovy tablet (1.5 mg rising to 25 mg). Common side effects

The common side effects are gastrointestinal. In the pooled Wegovy weight trials, nausea affected 44% on semaglutide 2.4 mg against 16% on placebo, and severe gastrointestinal reactions were reported in 4.1% against 0.9%. The detailed dose-by-dose figures are in our semaglutide side effects guide.

Warnings that apply to every semaglutide product

These come from the Wegovy label and apply equally to generic versions of the same molecule:

• Boxed warning: semaglutide causes thyroid C-cell tumours in rodents. It must not be used by anyone with a personal or family history of medullary thyroid cancer or with MEN 2.
• Allergy: do not use after a serious allergic reaction (such as anaphylaxis or angioedema) to semaglutide or any ingredient of the product.
• Pancreatitis: acute pancreatitis, including fatal cases, has occurred with GLP-1 drugs including semaglutide.
• Gallbladder disease: gallstones and gallbladder inflammation are more common than on placebo (gallstones in 1.6% against 0.7% in the adult weight trials).
• Kidney injury, sometimes needing dialysis, has followed dehydration from vomiting or diarrhoea.
• Diabetic eye disease: in people with type 2 diabetes, diabetic retinopathy complications were more frequent in trials, and anyone with existing retinopathy should be monitored.
• Heart rate: resting heart rate rose by 1 to 4 beats per minute on average; a sustained rise at rest is a reason to stop.
• Low blood sugar is more likely with insulin or a sulfonylurea.
• Anaesthesia: tell the surgical team, because slowed stomach emptying raises the risk of aspiration.
• Pregnancy: the label advises stopping at least 2 months before a planned pregnancy because of the long half-life.
• Do not combine semaglutide products with each other or with another GLP-1 drug.

Storage: the cold chain problem

Semaglutide injections are temperature sensitive, and this matters more than price when pens are shipped internationally. The Wegovy label says single-dose pens must be kept in a refrigerator at 2°C to 8°C before use, may be kept at 8°C to 30°C for up to 28 days if needed, must never be frozen, and must be kept in the carton away from light. The multi-dose FlexTouch pen can be kept for 56 days at 15°C to 30°C after first use.

A pen that has been frozen or left in a hot delivery van may look normal and still have lost potency. Before ordering any injectable from abroad, ask the seller how pens are packed for temperature, how long transit takes, and what happens if a parcel is delayed. When the pen arrives, check that the solution is clear and colourless, as the label describes, and do not use a pen that has been frozen.

Counterfeits are a real problem

Demand for semaglutide has made it a target. The FDA has warned about counterfeit Ozempic 1 mg pens found in the legitimate US supply chain three times: thousands of units seized in December 2023 (with counterfeit, possibly non-sterile needles), several hundred in April 2025 and dozens more in December 2025. In the 2025 cases the fakes carried genuine Novo Nordisk lot numbers. Outside regulated supply chains the risk is higher, and "research use only" semaglutide is not made to pharmaceutical standards at all.

When you receive any semaglutide pen, check:

1. The manufacturer is named and matches the brand (for example Sun Pharmaceutical Industries for Noveltreat, Novo Nordisk for Wegovy).
2. The carton has a batch number and expiry date, and they match the pen.
3. The strength on the pen matches your prescription and your current step in the dose schedule.
4. The pack arrived cold if it had not yet been opened.
5. It is a sealed pen, not a vial or powder labelled for research.

What semaglutide costs

US prices are the reference point most readers ask about. The Medicaid NADAC survey records what US pharmacies pay; CMS publishes the Medicare negotiated price that applies from 2027.

CMS set a single negotiated price of $274 for a 30-day supply across Ozempic, Rybelsus and Wegovy, 71% below the 2024 list price of $959, with package-level prices such as those above. That is what Medicare Part D plans pay, not what a patient pays at the counter. In India, Sun Pharma's stated weekly therapy cost of about ₹900 to ₹2,000 for Noveltreat works out to roughly ₹3,600 to ₹8,000 for four weeks.

To be plain about it: these are prescription medicines, semaglutide generics are not approved in the US, UK or EU, importing unapproved drugs into the US for personal use is illegal in most circumstances, and importing them into the UK, EU or Australia may also not be legal. Check your country's rules first, and use semaglutide only under a prescriber who can monitor you.

Other options if semaglutide does not fit

For type 2 diabetes, semaglutide is one of several drug classes.
