Amicline 500mg

Amicline 500mg is a tablet made in India by Franco-Indian Pharmaceuticals. Each tablet contains 500 mg of diloxanide furoate, a medicine that kills the amoeba Entamoeba histolytica inside the bowel. It is a luminal amoebicide: it works on the amoebae and cysts living in the gut, not on amoebae that have invaded the bowel wall or spread to the liver. We sell Amicline 500 in packs of 100, 200 and 300 tablets, from $0.30 a tablet in the largest pack.

Three things before you read further. First, Amicline is not a worm medicine. Some shop listings, including our own catalogue category, file it under worm treatments, but diloxanide is an antiprotozoal (WHO classification P01AC01) and has no use against worms. Second, it needs a diagnosis: amoebiasis is confirmed with stool tests, and a stool infection with blood, fever or a liver abscess needs a different medicine first. Third, a standard adult course is 30 tablets, and our smallest pack holds 100; read the price section before choosing a pack. This page uses the UK prescribing information for diloxanide 500 mg tablets, the WHO Model List of Essential Medicines, the Merck Manual, a Cochrane review and the Indian product listings, all listed at the end. We found no published trial of Amicline itself.

All three packs cost less than $149, so each one carries the $25 shipping charge when ordered alone. The larger packs are cheaper per tablet before shipping as well as after it.

How many tablets does a course need? The UK prescribing information gives adults one 500 mg tablet three times a day for ten days. That is 30 tablets for one adult course. Our smallest pack of 100 tablets is more than three adult courses, and the 200 and 300 packs are far more than one person needs for one infection. In India, Amicline is sold in strips of 10, so a pharmacy there would supply three strips for a course. We do not sell a 30 tablet pack. If you are treating one person once, the 100 pack is the only one that makes sense, and most of it will be left over. Please do not keep leftover tablets to treat yourself next time: a new bout of diarrhoea needs a new diagnosis, and diloxanide on its own does not treat the more serious forms of amoebiasis.

Compared with other treatments for amoebae in the gut

NADAC is the US National Average Drug Acquisition Cost, what US pharmacies pay for a medicine. There is no NADAC price for diloxanide because no commercial diloxanide product is marketed in the US (the Merck Manual says it may be obtained through some compounding pharmacies), and we found no current NADAC row for paromomycin either, so we cannot give a fair US cost comparison. The Merck Manual lists three options for clearing cysts from the bowel: iodoquinol for 20 days, paromomycin for 7 days and diloxanide furoate for 10 days. It notes that paromomycin is safe for use in pregnancy and that diloxanide furoate is not available commercially in the United States but may be obtained through some compounding pharmacies. If you are in the US or Canada and have a prescription, ask your pharmacist about paromomycin before buying diloxanide from abroad.

Which strength to choose

Amicline comes in one strength, 500 mg, the strength the WHO Model List of Essential Medicines lists for diloxanide (core list, 500 mg oral solid form).

The WHO list gives the same weight limit for children: over 25 kg. One Indian pharmacy listing (PharmEasy) says Amicline should not be used in children under 12 (Patient.info says the same); the UK prescribing information sets a weight limit, not an age limit. Either way, a child's dose must come from a doctor.

What diloxanide treats and what it does not

Entamoeba histolytica lives in two forms: cysts, which pass in stool and spread the infection, and trophozoites, which can invade the bowel wall and cause bloody diarrhoea (amoebic colitis or dysentery) or travel to the liver. Diloxanide acts on the amoebae inside the bowel.

• People who pass cysts but have no symptoms. The Merck Manual says asymptomatic people who pass E. histolytica cysts should be treated with paromomycin, iodoquinol or diloxanide furoate, to prevent invasive disease and spread to others.
• After treatment of amoebic colitis or a liver abscess. The Merck Manual advises tinidazole, metronidazole, secnidazole or ornidazole first, then a second medicine such as diloxanide to clear the cysts left in the bowel.
• Not on its own for invasive disease. The USP DI drug monograph states that diloxanide alone is not effective in the treatment of invasive or extraintestinal amoebiasis. Bloody diarrhoea, fever or pain under the right ribs needs a doctor and a different medicine first.
• Not for harmless look-alikes. Stool microscopy cannot tell E. histolytica from E. dispar, a harmless gut resident. The Merck Manual says treatment is not necessary for E. dispar, so ask for a test that identifies the species.

How good is the evidence? A 2019 Cochrane review of drugs for amoebic colitis (41 trials, 4,999 participants) found that, compared with metronidazole alone, combination therapy may result in fewer parasitological failures (3 trials, 720 participants; low-certainty evidence). The authors were uncertain which combination works best, did not reach conclusions about diloxanide on its own, and noted that most trials were small and more than 20 years old.

How to take Amicline 500mg

• Adults: one tablet three times a day, with meals, for ten days (UK prescribing information for the dose; USP DI monograph for taking it with meals).
• Swallow the tablet whole with a glass of water. The PharmEasy listing says not to cut, break or chew it.
• The USP DI monograph advises taking it with meals to reduce stomach upset.
• Finish the full ten days even if you feel better. The USP DI monograph lists completing the full course as important.
• If you miss a dose, take it when you remember unless the next dose is almost due; do not take two doses at once (USP DI monograph).
• Follow-up: the USP DI monograph says stool tests to check that treatment worked should be done no earlier than 2 weeks after the end of treatment.
• Wash your hands well after the toilet and before handling food, to protect the people you live with.
• Store below 25 °C in a cool, dry place (PharmEasy listing), away from children.

Side effects

No label gives percentages for diloxanide side effects, so we have not charted them.

Wind (flatulence) is the side effect most sources mention. Diarrhoea can come from the infection as well as the medicine; drink plenty of fluids, and if it gets worse rather than better, see a doctor.

When to get urgent help

Stop taking Amicline and get medical help straight away if you have:

• Swelling of the face, lips, tongue or throat, trouble breathing or swallowing, or a widespread itchy rash or hives. Allergy to diloxanide furoate is the contraindication in the UK prescribing information. Call emergency services (911 in the US and Canada, 999 in the UK, 000 in Australia).
• Blood in your stool, high fever, severe stomach pain, or pain under the right ribs. These can be signs of invasive amoebiasis or a liver abscess, which diloxanide alone does not treat.
• Signs of dehydration: very little urine, dizziness on standing, confusion, or a child who is drowsy or not drinking.

The UK prescribing information says diloxanide tablets are unlikely to constitute a hazard in overdosage, and there is no specific antidote. If someone has taken many tablets, contact a doctor or poison centre anyway, and tell emergency staff that the medicine was diloxanide furoate, how much was taken and when.

Who should not take Amicline 500mg

Never take it if:

• you are allergic to diloxanide furoate or any ingredient of the tablet (UK prescribing information, contraindications; the PharmEasy listing gives the same).
• you are pregnant or breastfeeding: the UK prescribing information states diloxanide is "Contraindicated in pregnancy and lactation." The USP DI monograph adds that studies in humans have not been done, though animal studies found no harm to the unborn young. The Merck Manual describes paromomycin as safe for use in pregnancy, so talk to your doctor about that option.
• it is for a child under 25 kg: the UK prescribing information says diloxanide is not suitable.

Do not use it on its own for bloody diarrhoea, amoebic dysentery or an amoebic liver abscess; these need a medicine such as metronidazole or tinidazole first.

Medicines that interact with Amicline

The UK prescribing information says no clinically significant drug interactions are known. Diloxanide is often prescribed after metronidazole or tinidazole. If your doctor gives you one of those as well, follow that medicine's own label, which carries its own warnings (for example about alcohol). Tell your doctor or pharmacist about every medicine and supplement you take.

Prescription and legal status

Amicline itself is not licensed by the FDA, the MHRA or Health Canada, and we could not check its status with the Australian TGA. In India it is a prescription medicine: PharmEasy marks Amicline 500 as requiring a prescription.

• US: the FDA's approvals database has no diloxanide product, and the Merck Manual says it is not available commercially in the United States. The FDA says personal importation of unapproved medicines is illegal in most circumstances.
• UK: diloxanide 500 mg tablets were licensed (PL 20072/0225, prescription only) but were discontinued in June 2018, according to Patient.info, which says no branded or generic products containing diloxanide were then available in the UK.
• Canada: Health Canada's Drug Product Database has no diloxanide product.
• Australia: we could not search the Australian Register of Therapeutic Goods, so we cannot tell you its status there. Ask a pharmacist.

Diloxanide is on the WHO Model List of Essential Medicines (core list, 500 mg oral solid form). Check the rules where you live.

Buying genuine Amicline

The MHRA warns: "Any medicine not authorised for sale in the UK can be unsafe or ineffective and there is no way of knowing what is in them or the negative health effects they can have." That warning covers Amicline, which is not authorised in the UK. The MHRA advises UK buyers to "Only use UK-registered pharmacies, including online pharmacies displaying the official green cross logo". When your order arrives, read the strip: it should name Franco-Indian Pharmaceuticals and diloxanide furoate 500 mg, with a batch number and an expiry date. These checks cannot prove that tablets are genuine, because counterfeiters copy packaging. If a batch causes unusual symptoms, seek medical advice and report it to your medicines regulator (the MHRA Yellow Card scheme in the UK, FDA MedWatch in the US, the TGA in Australia or Health Canada) as well as to us.
