Super Kamagra

Super Kamagra is a combination tablet made in India by Ajanta Pharma. Each tablet contains two medicines: sildenafil 100 mg, the active ingredient in Viagra, for erectile dysfunction, and dapoxetine 60 mg for premature ejaculation. It is often sold as "160 mg", which is the two doses added together, not 160 mg of anything. We sell it in packs of 36, 60, 84 and 96 tablets, from $1.98 a tablet in the largest pack.

Super Kamagra only makes sense for men who have both conditions. Who Super Kamagra is for, and who it is not for

Erectile dysfunction is difficulty getting or keeping an erection firm enough for sex. Premature ejaculation is ejaculating sooner than you want with little control. Sildenafil treats the first and does not delay ejaculation. Dapoxetine, a short-acting SSRI, treats the second and does nothing for erections; erection problems are listed among its own common side effects.

The official Priligy product information says dapoxetine should only be prescribed to men who meet all of these criteria:

• Time from penetration to ejaculation of less than two minutes.
• Persistent or recurrent ejaculation with minimal stimulation, before, on or shortly after penetration and before the man wishes.
• Marked personal distress or relationship difficulty because of it.
• Poor control over ejaculation.
• Premature ejaculation in most attempts over the previous six months.

It also says dapoxetine should not be used to delay ejaculation in men who have not been diagnosed with premature ejaculation. Some men with weak erections rush to finish before the erection fades, which looks like premature ejaculation but is an erection problem; treating the erection alone may be enough.

What regulators and the dapoxetine label say about the combination

Neither the FDA, the MHRA, the European Medicines Agency, the TGA nor Health Canada has approved a sildenafil and dapoxetine combination tablet, and the FDA has not approved dapoxetine on its own. India's Central Drugs Standard Control Organisation approved sildenafil and dapoxetine tablets (100 mg with 60 mg, 50 mg with 60 mg, and 50 mg with 30 mg) in February 2020, for men aged 18 to 64 who have both conditions.

The European Priligy product information says dapoxetine should not be used in men taking PDE5 inhibitors such as sildenafil, because of possible reduced tolerance to standing up, and that the efficacy and safety of the two together have not been established. In its interaction study, sildenafil 100 mg raised dapoxetine exposure by 22% and its peak by 4%, which the label does not expect to be clinically significant; the concern is the combined effect on blood pressure.

One trial offers some reassurance. In a 12 week trial of 495 men with both conditions who were already on a stable PDE5 inhibitor, adding dapoxetine raised the average time to ejaculation to 5.2 minutes against 3.4 minutes on placebo, with side effects in 29.6% against 20.0%. That trial used separate tablets, flexible dapoxetine doses of 30 or 60 mg, and men whose erections were already working on their PDE5 inhibitor. The product information was last revised in 2021, after this trial, and still says the safety and efficacy of the combination have not been established. Why 60 mg of dapoxetine is not a starting dose

The Priligy product information sets the starting dose for every patient at 30 mg and says treatment should not be started with 60 mg. The dose can go up to 60 mg only if 30 mg did not work well enough and the man had no moderate or severe side effects and no warning signs of fainting. Every Super Kamagra tablet contains 60 mg, so taking one as a first dose breaks that rule. The sildenafil side is also at its maximum: the label's usual starting dose is 50 mg.

The extra 30 mg buys little at the median. In a 24 week placebo-controlled trial of 1,162 men with premature ejaculation, the median average time to ejaculation at the end was:

How to take Super Kamagra

• Take one tablet whole with water about an hour before sex. Sildenafil works from about 30 minutes to 4 hours after a dose; dapoxetine is taken 1 to 3 hours before sex and peaks 1 to 2 hours after the tablet, so about an hour suits both.
• Take no more than one tablet in 24 hours. Both drugs share that limit, and dapoxetine is not for continuous daily use; the label asks a doctor to review whether it is worth continuing after the first four weeks or six doses.
• Do not drink alcohol with it. The Priligy product information says patients should not combine the two, because alcohol can increase dizziness, drowsiness, slowed reflexes and impaired judgement, and may make fainting more likely.
• Do not take it with recreational drugs. Ecstasy (MDMA), ketamine and LSD can cause serious reactions with dapoxetine, including heart rhythm problems, overheating and serotonin syndrome. Opioids and benzodiazepines add to drowsiness and dizziness.
• Avoid grapefruit juice for 24 hours before a dose. It raises dapoxetine levels.
• An empty or light stomach helps the sildenafil; a high-fat meal delays its peak by about an hour. Food makes little difference to dapoxetine.
• Sildenafil works only with sexual stimulation.
• Never combine it with any other erectile dysfunction treatment, including tadalafil, vardenafil, avanafil or another sildenafil product, or with another dapoxetine product.

Lower doses or no Super Kamagra for some men

For sildenafil, the label advises considering a 25 mg starting dose for men who are over 65, have liver impairment or severe kidney impairment, or take erythromycin or a strong CYP3A4 inhibitor; men on an alpha blocker should be stable on it and start at 25 mg; men taking ritonavir should take no more than 25 mg in 48 hours. For dapoxetine, strong CYP3A4 inhibitors are a contraindication, and moderate ones such as erythromycin, clarithromycin, fluconazole, amprenavir, fosamprenavir, aprepitant, verapamil and diltiazem limit the dose to 30 mg. Dapoxetine is not recommended in severe kidney impairment and needs caution in mild or moderate impairment. The label also asks for caution at 60 mg in men known to be poor CYP2D6 metabolisers and in men taking a strong CYP2D6 inhibitor, because dapoxetine levels can rise; ask a pharmacist if you are unsure about a medicine. A tablet with 100 mg of sildenafil and 60 mg of dapoxetine is the wrong product for all of these men.

Side effects of each drug

Dapoxetine's common side effects rise sharply from 30 mg to 60 mg. The product information gives these rates without matching placebo figures:

These come on top of sildenafil's own side effects. In the fixed-dose trials in the sildenafil label, men on 100 mg reported headache (28% against 7% on placebo), flushing (18% against 2%), indigestion (17% against 2%), abnormal vision, mostly a colour tinge (11% against 1%), and nasal congestion (9% against 2%). The fainting risk dapoxetine adds

Dapoxetine can cause fainting, sometimes with a very slow heartbeat or a pause in the heart rhythm. In the placebo-controlled phase 3 trials, loss of consciousness was reported in 0.06% of men on 30 mg and 0.23% on 60 mg, and in 0.64% of healthy volunteers across early studies. Most cases happened in the first 3 hours after a dose, often after the first dose, and were often preceded by nausea, dizziness, light-headedness or sweating. Fainting can also happen with no warning at all.

If you feel any of these warning signs, lie down straight away with your head low, or sit with your head between your knees, until they pass. Do not drive or use machinery. Drink enough fluids, and get up slowly after lying or sitting for a long time. The product information asks doctors to check blood pressure and pulse lying and standing before starting dapoxetine, and says it should be avoided in men who have had dizziness or fainting on standing up. Sildenafil also lowers blood pressure a little, which is why the dapoxetine label says it should not be used with PDE5 inhibitors such as sildenafil.

When to get urgent help

• Fainting, or chest pain, severe dizziness or nausea during or after sex.
• An erection lasting more than 4 hours. Without prompt treatment it can permanently damage the penis and cause lasting erectile dysfunction.
• Sudden loss or reduction of vision in one or both eyes, or sudden loss of hearing. Stop taking it and get medical help straight away.
• Agitation, confusion, fever, sweating or muscle twitching after taking it with another serotonin-acting medicine or a recreational drug such as ecstasy (possible serotonin syndrome).
• A seizure. Stop taking it.
• Swelling of the lips, tongue or throat, difficulty breathing or swallowing, or a rash, hives or blistering skin.
• If you take more than one tablet, contact a doctor or emergency department straight away.

If you need emergency care for a heart problem, tell the staff that you took sildenafil and dapoxetine, how much and when. Nitrates, a standard emergency treatment for chest pain, must not be given soon after a sildenafil dose; the label says it is not known when they can safely be given, even 24 hours later.

Who must not take Super Kamagra

Everyone who must not take sildenafil, plus everyone who must not take dapoxetine.

• Anyone using nitrates in any form, including nitroglycerin and amyl or butyl nitrite "poppers", or a guanylate cyclase stimulator such as riociguat or vericiguat.
• Anyone allergic to sildenafil, dapoxetine or any ingredient of the tablet.
• Men with heart failure (NYHA class II to IV), heart conduction problems such as AV block or sick sinus syndrome, significant ischaemic heart disease or significant heart valve disease (dapoxetine contraindications).
• Men with a history of fainting, or who have had dizziness or fainting on standing up.
• Men with a history of mania, hypomania, bipolar disorder or severe depression, men with current depression, and men with another psychiatric condition such as schizophrenia. Do not stop an antidepressant in order to take this.
• Men with epilepsy that is not well controlled.
• Men taking an SSRI, SNRI or tricyclic antidepressant, an MAOI, thioridazine, or other serotonin-acting medicines such as triptans, tramadol, linezolid, lithium, L-tryptophan or St John's wort, or within 14 days of stopping one. Do not start one of these within 7 days of your last dose.
• Men taking a strong CYP3A4 inhibitor such as ketoconazole, itraconazole, ritonavir, saquinavir, telithromycin, nefazodone, nelfinavir or atazanavir.
• Men with moderate or severe liver impairment.
• Men with blood pressure below 90/50, a recent heart attack or stroke, retinitis pigmentosa or another inherited retinal disorder, or previous loss of vision in one eye from NAION (contraindications in the UK and EU sildenafil label; the US label advises caution).
• Men for whom sex is inadvisable because of a heart condition.

Dapoxetine is indicated only for men aged 18 to 64. Talk to a doctor first if you have aortic stenosis, a deformed penis, a condition that raises the risk of prolonged erections (sickle cell anaemia, multiple myeloma, leukaemia), controlled epilepsy, raised eye pressure or a risk of angle-closure glaucoma, a bleeding disorder or an active stomach ulcer. The same applies if you take warfarin, aspirin, anti-inflammatory painkillers such as ibuprofen or other blood-thinning medicines, blood pressure medicines, or medicines that act on the brain such as antiepileptics, antipsychotics, anti-anxiety medicines or sleeping tablets. Super Kamagra does not protect against sexually transmitted infections.

Prescription and legal status

Sildenafil is a prescription medicine in the US, Australia and Canada, and in the UK apart from 50 mg pharmacy packs. Dapoxetine is licensed as Priligy, on prescription, in the UK, Australia and much of Europe, but not by the FDA or Health Canada. No sildenafil and dapoxetine combination is licensed by the FDA, the MHRA, the TGA or Health Canada, and Super Kamagra is not licensed in any of these countries. In the US, the FDA says personal importation of unapproved medicines is illegal in most circumstances. We recommend seeing a doctor before using either drug, and checking the rules where you live before ordering.

Buying genuine Super Kamagra

The MHRA warns that any medicine not authorised for sale in the UK can be unsafe or ineffective, and that erectile dysfunction pills bought online may contain no active ingredient, the wrong dose, hidden drugs or toxic ingredients. That warning covers Super Kamagra, which is not authorised in the UK, and the MHRA advises UK buyers to use UK-registered pharmacies. When your order arrives, check that each strip names Ajanta Pharma and shows both strengths, a batch number and an expiry date, and reject strips that are damaged, unsealed or unmarked. These checks cannot prove a product is genuine on their own, because counterfeiters copy packaging. If a batch causes unusual symptoms, stop using it, seek medical advice, and report it to your medicines regulator (the MHRA Yellow Card scheme in the UK, FDA MedWatch in the US, the TGA in Australia or Health Canada) as well as to us.
