Premature Ejaculation: What It Is and What Actually Helps

What is Premature Ejaculation?
Premature ejaculation means ejaculating sooner than you or your partner would like, with little control over timing, in a way that causes distress. Estimates of how common it is vary with the definition used, but reviews commonly put it at around 20 to 30 percent of men, which makes it at least as common as erectile dysfunction. Doctors separate two main types. Lifelong premature ejaculation has been present since a man first became sexually active, with ejaculation typically within about a minute of penetration. Acquired premature ejaculation develops later after a period of normal control, and it is more often linked to another problem, such as erectile dysfunction, prostate inflammation, thyroid disease or anxiety. In clinical trials, the measure used is intravaginal ejaculatory latency time: the time from penetration to ejaculation, timed with a stopwatch. The dapoxetine trials enrolled men whose time was two minutes or less, who felt they had poor control, and who were distressed by it. Time alone is not the diagnosis; control and distress matter just as much.

Treatment Options
Dapoxetine is the only medicine developed specifically for premature ejaculation. It is a short-acting SSRI, taken one to three hours before sex, starting at 30 mg with a maximum of 60 mg once in 24 hours. In the 24 week Priligy trial, median ejaculation time rose from 1.05 minutes on placebo to 1.72 minutes on 30 mg and 1.91 minutes on 60 mg, and around two thirds of men on 60 mg said their condition was at least slightly better. Nausea and dizziness are the most common side effects, and fainting is a rare risk that is highest after the first dose. Other options include daily SSRIs used off label, which can have larger effects but mean daily side effects; topical anaesthetic sprays and creams; and behavioural methods such as the stop-start and squeeze techniques. When erectile dysfunction is also present, treating it with a PDE5 inhibitor such as sildenafil or tadalafil often helps, and combination tablets containing sildenafil or tadalafil with dapoxetine are widely used, although the official dapoxetine label advises caution with that pairing. Treatment gives control while it is used; premature ejaculation usually returns when medicines are stopped.
