Filitra 40mg

Filitra 40 is a vardenafil tablet made in India by Fortune Healthcare. Each tablet contains 40 mg of vardenafil, twice the 20 mg maximum in the vardenafil label. No regulator in the US, UK, EU, Australia or Canada has approved vardenafil above 20 mg. We sell it in packs of 60, 90 and 120 tablets, from $1.09 a tablet.

We stock Filitra 40 because customers ask for it by name, but it is not a starting dose and we would not recommend it first or second. This page covers the price per tablet, what the vardenafil label says about doses above 20 mg, which strength suits most men, how to take it, the side effects, who must not take it, and how delivery works. No pack reaches the $149 free shipping threshold on its own; the 120 pack is $18 short. For comparison, US pharmacies paid about $2.79 per FDA-approved generic vardenafil 20 mg tablet as of 30 September 2026 in the Medicaid NADAC survey; the brand, Levitra, is no longer marketed in the US. What the label says about doses above 20 mg

Vardenafil is approved for erectile dysfunction at 2.5 mg to 20 mg, and the label's efficacy and side effect data cover 5 mg to 20 mg. Higher doses appear only in the overdose, warnings and clinical pharmacology sections:

• Single doses up to 80 mg, and 40 mg once daily for 4 weeks, were tolerated without serious side effects.
• When 40 mg was given twice daily, cases of severe back pain were seen.
• Most volunteers given a single 120 mg dose had reversible back pain or muscle pain, abnormal vision, or both.
• In 25 healthy men, a single 40 mg dose did not change visual acuity, eye pressure or eye examination findings.
• In a heart rhythm study of 59 healthy men, both the usual 10 mg dose and a single 80 mg dose raised the corrected QT interval about as much as the antibiotic moxifloxacin, a drug known to prolong it. The clinical impact is unknown, and the effect adds to that of other medicines that prolong QT. The label tells men with long QT syndrome, or who take class IA or class III antiarrhythmics such as quinidine, procainamide, amiodarone or sotalol, to avoid vardenafil.

These were small studies, mostly in healthy volunteers. They do not show that 40 mg is safe for men with heart disease, men aged 65 or over, or men taking other medicines, and they are not evidence that 40 mg works better than 20 mg. The label's placebo-controlled trials suggested that headache, flushing, indigestion, nausea and nasal congestion became more common as the dose rose from 5 mg to 20 mg.

One small published crossover trial in 21 men with erectile dysfunction did test 40 mg. Erection measurements improved over placebo by similar amounts on 20 mg and 40 mg, the two doses were not compared statistically, and side effects, mainly headache, flushing and nasal congestion, were slightly more common on 40 mg than on placebo.

Which strength to choose

The label recommends 10 mg for most men, allows 20 mg or 5 mg depending on effect and side effects, and advises considering 5 mg to start for men aged 65 or over.

If 20 mg is not working, check the practical causes first: judging it too soon, too little sexual stimulation, heavy drinking, or a worsening cause such as diabetes or vascular disease. Some men respond better to a different drug, such as tadalafil (Vidalista 20) or sildenafil (Cenforce 100). Erectile dysfunction that does not respond to the labelled dose is a reason to see a doctor, because it can be an early sign of heart disease or diabetes.

If you and your doctor decide on Filitra 40

• Only consider it if a doctor has looked at why 20 mg is not working and agrees to a higher dose. 40 mg is above the labelled maximum, and it has not been shown to work better than 20 mg.
• Take one tablet about an hour before sex. On an empty stomach, peak levels usually arrive between 30 minutes and 2 hours after a dose (median 60 minutes). It can be taken with or without food, though a high-fat meal lowered the peak level by 18% to 50%.
• Take no more than one tablet in 24 hours, and never combine it with another vardenafil, sildenafil, tadalafil or avanafil product.
• It works only with sexual stimulation.
• If side effects are troublesome, stop and speak to your doctor.

Men for whom 40 mg is plainly the wrong product

Vardenafil is cleared by the liver enzyme CYP3A4, and some medicines raise its levels many times over. The label's limits are far below 40 mg:

• Ritonavir or cobicistat: no more than 2.5 mg in 72 hours.
• Indinavir, saquinavir, atazanavir, clarithromycin, or ketoconazole or itraconazole at 400 mg a day: no more than 2.5 mg in 24 hours.
• Erythromycin, or ketoconazole or itraconazole at 200 mg a day: no more than 5 mg in 24 hours.
• Alpha blockers: be stable on the alpha blocker first, then start at 5 mg.
• Blood pressure medicines: vardenafil adds to their effect. In a label study, 20 mg lowered lying-down blood pressure by an average of 7/8 mmHg more than placebo.
• Grapefruit juice: likely to raise vardenafil levels.
• Moderate liver impairment: start at 5 mg and take no more than 10 mg. Severe liver impairment or kidney dialysis: do not use vardenafil.
• Age 65 or over: overall exposure was about 52% higher, and peak levels about 34% higher, than in men aged 18 to 45; consider 5 mg to start.

Side effects at the labelled doses

These rates come from the placebo-controlled trials of vardenafil 5 mg to 20 mg. The label gives no figures for 40 mg.

In the trials, 3.4% of men on vardenafil stopped because of side effects, against 1.1% on placebo. Vardenafil can also cause temporary colour vision changes, such as a blue tinge or difficulty telling blue from green. The label describes this effect as dose related, which matters at twice the maximum dose.

When to get urgent help

• An erection lasting more than 4 hours. Without prompt treatment it can permanently damage the penis and cause lasting erectile dysfunction.
• Sudden loss or reduction of vision in one or both eyes. Stop taking vardenafil and get medical help straight away.
• Sudden loss or reduction of hearing, sometimes with ringing in the ears or dizziness. Stop and get prompt medical attention.
• Chest pain, fainting, palpitations or severe dizziness during or after sex.
• Swelling of the lips, tongue or throat, difficulty breathing or swallowing, or a rash, hives or blistering skin.
• If you take more than one tablet, contact a doctor or emergency department straight away.

Sudden vision loss from NAION, a problem with blood flow to the optic nerve, has been reported rarely with all PDE5 inhibitors. Observational studies suggest about a 2-fold higher risk shortly after a dose, though they do not prove the drugs cause it. The risk is higher in men who have had NAION before, and most reported cases had risk factors such as a "crowded" optic disc, age over 50, diabetes, high blood pressure, coronary artery disease, high cholesterol or smoking.

If you need emergency care for a heart problem, tell the staff that you took vardenafil, how much and when. The label says a safe interval between vardenafil and nitrates has not been determined.

Who should not take Filitra 40

Never take it if:

• You use nitrates or nitric oxide donors in any form, regularly or occasionally, including nitroglycerin and amyl or butyl nitrite "poppers".
• You take a guanylate cyclase stimulator such as riociguat (Adempas) or vericiguat (Verquvo).
• You have had an allergic reaction to vardenafil (a contraindication in the UK and EU product information).
• You have lost vision in one eye from NAION (a sudden loss of blood flow to the optic nerve). This is a contraindication in the EU product information; the US label says to use vardenafil with caution, and only when the expected benefits outweigh the risks.

The US label says these men should avoid vardenafil, should not use it, or are not recommended to take it:

• Men with long QT syndrome or who take class IA or class III antiarrhythmics.
• Men for whom sex is inadvisable because of a heart condition. The label does not recommend vardenafil for men with unstable angina, resting systolic blood pressure below 90 mmHg, uncontrolled high blood pressure (above 170/110), a stroke, life-threatening arrhythmia or heart attack in the last 6 months, or severe heart failure.
• Men with severe liver impairment, on kidney dialysis, or with hereditary retinal disorders such as retinitis pigmentosa.

The EU product information goes further. Because vardenafil has not been studied in these groups, it lists them as contraindications: severe liver impairment (Child-Pugh C), kidney failure needing dialysis, blood pressure below 90/50 mmHg, a stroke or heart attack in the last 6 months, unstable angina, and hereditary retinal disorders such as retinitis pigmentosa. It also contraindicates vardenafil with ritonavir or indinavir, and with ketoconazole or itraconazole tablets in men over 75.

Do not combine it with other erectile dysfunction treatments. Talk to a doctor first if you have aortic stenosis or another obstruction to blood flow out of the heart, a deformed penis (angulation or Peyronie's disease), a condition that raises the risk of prolonged erections (sickle cell anaemia, multiple myeloma, leukaemia), pulmonary hypertension, hearing problems, a previous erection that lasted more than 4 hours, a past seizure, a bleeding disorder or an active stomach ulcer. Filitra is for adult men only, and it does not protect against sexually transmitted infections.

Prescription and legal status

Vardenafil is a prescription medicine in the US, UK, Australia and Canada. No vardenafil product is approved at 40 mg by the FDA, the EMA, the MHRA, the TGA or Health Canada, and Filitra is not licensed by any of them. In the US, the FDA says personal importation of unapproved medicines is illegal in most circumstances. We recommend seeing a doctor before using vardenafil, especially at a dose above 20 mg, and checking the rules where you live before ordering.

Buying genuine Filitra

The MHRA warns that any medicine not authorised for sale in the UK, which includes Filitra, can be unsafe or ineffective, that there is no way of knowing what is in them, and that erectile dysfunction pills bought online may contain no active ingredient, the wrong dose, hidden drugs or toxic ingredients. It advises UK buyers to use only UK-registered pharmacies. When your order arrives, check that each strip names Fortune Healthcare and shows a batch number and an expiry date, and reject strips that are damaged, unsealed or unmarked. These checks cannot prove a product is genuine on their own, because counterfeiters copy packaging. If a batch causes unusual symptoms, stop using it, seek medical advice, and report it to your medicines regulator (the MHRA Yellow Card scheme in the UK, FDA MedWatch in the US, the TGA in Australia or Health Canada) as well as to us.
