Duratia 60mg

Duratia 60mg is a dapoxetine 60 mg tablet for diagnosed premature ejaculation, sold under the Duratia brand from Fortune Healthcare, an Indian company. It is a second-step strength, not a first one. The Priligy product information sets 60 mg as the maximum dose and says "Treatment with Priligy should not be initiated with the 60 mg dose"; every man starts on 30 mg, and a move to 60 mg is your doctor's decision, made only under the conditions set out below. If you have never taken dapoxetine, the tablet to start with is Duratia 30mg. The FDA and Health Canada have not approved dapoxetine, and Duratia is not approved by the FDA, MHRA, TGA or Health Canada. Below we cover the label's rules for stepping up to 60 mg, what 60 mg added over 30 mg in the main trial, the extra side effects that come with it, who must not take it, and what each pack costs per tablet. All four packs cost less than $149 on their own, so a one-pack order pays $25 for shipping. Leave shipping aside and the 90 pack is the cheapest per tablet, a cent below the 60 and 120 packs. Add the $25 and the order flips: the 120 pack comes out lowest at $1.16, only because the fixed charge is divided across more tablets. Pack for pack, a Duratia 60mg tablet costs 7 or 8 cents more than a Duratia 30mg tablet, which runs from $0.87 to $1.00 before shipping.

Before you buy a large pack, keep in mind how the label expects 60 mg to be used. It is taken only when sex is planned, never more than once in 24 hours, and your doctor should review after the first four weeks or at least six doses whether the benefit outweighs the side effects, which are more frequent at 60 mg. Even at the label's limit of one tablet in 24 hours, 120 tablets would last at least four months; taken only when sex is planned, as the label intends, far longer. If you are still waiting for that first review, a smaller pack carries less risk of paying for tablets you stop using. Check the expiry date on the strips when your order arrives.

Compared with Priligy and Dapoforce 60mg

Priligy is the original licensed dapoxetine brand, made in 30 mg and 60 mg strengths. Because the FDA has not approved dapoxetine, there is no US brand, no US generic and no NADAC pharmacy cost to set against Duratia. The comparisons that matter are the licensed brand and Dapoforce 60mg, another Indian 60 mg brand we sell at the same prices.

Priligy earned its licence on clinical trial data. We have not found any published study that compares Duratia with Priligy or with another dapoxetine brand, so we cannot claim that the same amount of dapoxetine reaches the blood. Dapoforce 60mg is priced the same as Duratia 60mg at every pack size, and we have no evidence that either works better. Treat them as the same dose, and never take both. We found Duratia on Indian trade and export listings, but not on the main Indian retail pharmacy sites we checked (PharmEasy, Apollo and Truemeds, and a search of Tata 1mg). In the UK or Australia, a doctor can prescribe licensed Priligy.

Do you have premature ejaculation?

The Priligy product information limits dapoxetine to men with diagnosed premature ejaculation who meet all of these: ejaculation less than two minutes after penetration; persistent or recurrent ejaculation with minimal stimulation, before, on or shortly after penetration and before the man wishes; marked distress or relationship difficulty because of it; poor control over ejaculation; and premature ejaculation in most attempts over the last six months. It says dapoxetine should not be prescribed simply to delay ejaculation in men without that diagnosis, and that other forms of sexual dysfunction, including erectile dysfunction, should be carefully investigated first. Some men with weak erections rush to finish before the erection fades, which is an erection problem rather than premature ejaculation. Dapoxetine is not a treatment for erection problems, and erectile dysfunction is listed among its own common side effects.

Which strength to choose

Do not split a 60 mg tablet to make a 30 mg dose. The label says the tablets should be swallowed whole, and there is no evidence that a split tablet gives an accurate dose. If you are starting treatment, use a 30 mg tablet.

Moving from 30 mg to 60 mg

The Priligy product information allows 60 mg only when all of these apply, and the decision is your doctor's, not one to make yourself:

• Your response to 30 mg was not enough.
• You had no moderate or severe side effects on 30 mg.
• You had no warning signs of fainting on 30 mg, such as light-headedness, nausea or sweating soon after a dose. If you had an orthostatic reaction (a drop in blood pressure on standing) on 30 mg, the label says no dose escalation to 60 mg should be performed.
• Your doctor has checked that no group below applies to you. Men taking moderate CYP3A4 inhibitors stay at 30 mg, and the label advises caution before increasing to 60 mg in CYP2D6 poor metabolisers or men taking a potent CYP2D6 inhibitor.

Having used dapoxetine before does not, on its own, qualify you for 60 mg, and you should not take two 30 mg tablets together to make 60 mg. The label states that "The incidence and severity of adverse events is higher with the 60 mg dose." If side effects on 60 mg are troublesome, stop and speak to your doctor.

What 60 mg added in the trials

The Priligy product information reports a 24 week placebo-controlled trial of 1,162 men, 389 of whom took 60 mg as needed. At the end of the study the median average time to ejaculation was 1.05 minutes on placebo, 1.72 minutes on 30 mg and 1.91 minutes on 60 mg; the average (least squares mean) was 1.7, 2.9 and 3.3 minutes. 23.2% of men on placebo, 44.4% on 30 mg and 47.9% on 60 mg reached an average of at least 2 minutes. At week 24, 35.5% of men on 60 mg rated their condition "better" or "much better", against 28.4% on 30 mg and 14% on placebo. The label says results varied a lot between men. Men with erectile dysfunction were excluded from these trials.

So in this trial, moving from 30 mg to 60 mg, the median rose from 1.72 to 1.91 minutes and the average by 0.4 minutes, a smaller gain than the step from placebo to 30 mg. At 3 minutes or more the gap was wider: 37.4% on 60 mg against 26.0% on 30 mg (Table 3). Across the pooled phase 3 trials (section 4.8), nausea and dizziness were roughly twice as common at 60 mg as at 30 mg, and fainting was more common too (see below). How to take Duratia 60mg

1. Swallow one tablet whole with at least one full glass of water, about 1 to 3 hours before sex, with or without food. The label says swallowing it whole avoids the bitter taste.
2. Take it only when sex is planned. It is not for continuous daily use, and you must not take it more than once in 24 hours.
3. Do not drink alcohol. The label says combining alcohol with dapoxetine may increase alcohol's effects on thinking and reflexes and make fainting more likely, raising the risk of injury.
4. Do not take it with recreational drugs. Ketamine, ecstasy (MDMA) or LSD may lead to serious reactions such as heart rhythm problems, overheating and serotonin syndrome; opioids and benzodiazepines can add to drowsiness and dizziness.
5. Avoid grapefruit juice for 24 hours before a dose. The label calls it a potent CYP3A4 inhibitor, which raises dapoxetine levels.
6. Do not combine it with erectile dysfunction tablets such as sildenafil, tadalafil, vardenafil or avanafil, or with any other dapoxetine product (see below).
7. Drink enough fluids, and do not stand up quickly after lying or sitting for a long time.

Your doctor should check your blood pressure and pulse lying and standing before you start dapoxetine, and review whether to continue after the first four weeks or at least six doses, then at least every six months. The label says data beyond 24 weeks are limited.

Groups for whom Duratia 60mg is not suitable

• Moderate CYP3A4 inhibitors such as erythromycin, clarithromycin, fluconazole, amprenavir, fosamprenavir, aprepitant, verapamil and diltiazem: the label restricts dapoxetine to 30 mg and advises caution, so Duratia 60mg is not suitable. Potent CYP3A4 inhibitors are a contraindication (see below).
• CYP2D6 poor metabolisers, or men taking a potent CYP2D6 inhibitor: the label advises caution before increasing to 60 mg, because levels can be much higher and side effects more frequent and severe.
• Kidney impairment: not recommended in severe kidney impairment, so Duratia 60mg is not suitable then; caution in mild or moderate impairment.
• Liver impairment: dapoxetine must not be taken with moderate or severe liver impairment (Child-Pugh B or C).
• Alpha blockers and nitrates (for example tamsulosin, doxazosin or a nitroglycerin spray): the label says dapoxetine should be prescribed with caution, because of a possible drop in blood pressure on standing.
• Epilepsy medicines, antipsychotics, anxiety medicines and sleeping tablets (not antidepressants, which must not be combined; see below), and blood thinners or medicines that affect platelets (warfarin, aspirin, anti-inflammatory painkillers, antiplatelet medicines): the label advises caution.
• Age 65 and over: the label says dapoxetine's efficacy and safety have not been established in this group, and it is indicated only up to age 64.

Side effects by dose

The rates below come from the pooled phase 3 trials of dapoxetine. The label reports them as dose related and gives no matching placebo figures. Duratia 60mg sits in the 60 mg column.

At 60 mg, about one man in five had nausea and about one in ten had dizziness. Nausea and dizziness were the side effects that most often made men stop treatment. Other common side effects in the label include anxiety, agitation, restlessness, abnormal dreams, lower sex drive, drowsiness, poor concentration, tremor, pins and needles, blurred vision, ringing in the ears, flushing, sinus congestion, yawning, vomiting, constipation, stomach pain, indigestion, dry mouth, sweating, erectile dysfunction, irritability and raised blood pressure. Fainting

Dapoxetine can cause fainting, sometimes with a very slow heartbeat or a pause in the heartbeat. In the placebo-controlled phase 3 trials it happened in 0.23% of men on 60 mg, about 23 in 10,000, against 0.06% on 30 mg, and the label says fainting appears dose dependent. In the trials, most fainting happened within the first 3 hours after a dose, after the first dose, or during clinic procedures such as blood tests and blood pressure checks. Nausea, dizziness, light-headedness, palpitations, weakness, confusion or sweating often came first, but you can faint at any time, with or without these warning signs. If you notice them, lie down with your head lower than your body, or sit with your head between your knees, until they pass, and do not drive or use dangerous machinery. Men with heart disease were excluded from the trials, and the label says the risk of harm from fainting is higher in men with structural heart disease.

When to get urgent help

• Fainting, or chest pain, a racing or very slow heartbeat, or severe dizziness after a dose.
• Agitation, confusion, fever, sweating or muscle twitching, especially after taking it with another serotonin-acting medicine or a recreational drug (possible serotonin syndrome).
• A seizure. Stop taking it.
• Sudden eye pain or blurred vision. The label links dapoxetine to eye pain and widened pupils, and asks for caution in men at risk of angle-closure glaucoma.
• Low mood, thoughts of harming yourself, or unusually high mood and overactivity. Stop taking it and speak to a doctor.
• Signs of an allergic reaction: swelling of the lips, tongue or throat, difficulty breathing or swallowing, or a rash, hives or blistering skin.
• If you take more than one tablet in 24 hours, get medical advice straight away. The label reports no overdose cases, and lists drowsiness, nausea and vomiting, a fast heartbeat, tremor, agitation and dizziness as typical of SSRI overdose.

Tell emergency staff that you took dapoxetine, how much and when, and anything else you took with it.

Who should not take Duratia 60mg

Never take it if any of these apply (contraindications in the Priligy label):

• You are allergic to dapoxetine or any ingredient of the tablet.
• You have heart failure (NYHA class II to IV), heart conduction problems such as AV block or sick sinus syndrome, significant ischaemic heart disease or significant heart valve disease, or a history of fainting.
• You have had mania or severe depression.
• You take an MAOI, thioridazine, an antidepressant (SSRI, SNRI or tricyclic), or another serotonin-acting medicine or herbal product such as L-tryptophan, triptans, tramadol, linezolid, lithium or St John's wort, or stopped one within the last 14 days. Do not start one of these within 7 days of your last dose.
• You take a potent CYP3A4 inhibitor such as ketoconazole, itraconazole, ritonavir, saquinavir, telithromycin, nefazodone, nelfinavir or atazanavir.
• You have moderate or severe liver impairment.

The Priligy label also says dapoxetine should not be used, should be avoided, or is not recommended in these situations:

• With erectile dysfunction tablets. Dapoxetine should not be used in men with erectile dysfunction who are using PDE5 inhibitors such as sildenafil or tadalafil, because together they may cause a drop in blood pressure on standing, and the safety and efficacy of the combination have not been established. Do not combine Duratia with any ED tablet.
• After a documented or suspected drop in blood pressure on standing; treatment should be avoided.
• With a history of mania, hypomania or bipolar disorder, with schizophrenia, or with current depression. Do not stop an antidepressant in order to take dapoxetine, and stop and see a doctor if low mood develops.
• Under 18 years of age. It is indicated only from 18 to 64.
• With epilepsy that is not well controlled; stop after any seizure. Men with controlled epilepsy should be carefully monitored.
• With severe kidney impairment, where the label says dapoxetine is not recommended.
• With rare hereditary problems of galactose intolerance, total lactase deficiency or glucose-galactose malabsorption, because Priligy tablets contain lactose. We do not have the full ingredient list for Duratia, so check the pack.

Talk to a doctor first if you have raised eye pressure or a risk of angle-closure glaucoma, a bleeding or clotting disorder, mild or moderate kidney impairment, or a heart condition not listed above. Duratia does not protect against sexually transmitted infections.

Prescription and legal status

Dapoxetine is prescription-only in the UK and Australia, where it is licensed as Priligy. The FDA and Health Canada have not approved dapoxetine. Duratia is not licensed by the FDA, MHRA, TGA or Health Canada. In the US, the FDA says personal importation of unapproved medicines is illegal in most circumstances. See a doctor before using it and check the rules where you live.

Buying genuine Duratia

The MHRA warns that "any medicine not authorised for sale in the UK can be unsafe or ineffective and there is no way of knowing what is in them or the negative health effects they can have". Duratia has no UK authorisation, so the warning applies to it, and the MHRA tells UK buyers to use only UK-registered pharmacies. When the parcel arrives, look for the Duratia name, the 60 mg strength, the maker, a batch number and an expiry date on every strip, and do not use strips that are torn, unsealed or unmarked. Passing these checks does not prove a tablet is genuine, since counterfeiters copy packaging too. If a batch causes unusual symptoms, stop using it, seek medical advice, and report it to your medicines regulator (the MHRA Yellow Card scheme in the UK, FDA MedWatch in the US, the TGA in Australia or Health Canada) as well as to us.
